Dry Eye After LASIK: What to Expect?

LASIK has one of the highest patient satisfaction rates of any elective surgery, but it's also honest to say that temporary dry eye is one of the most common side effects patients experience afterward. If you're considering LASIK, or you've recently had it and your eyes feel drier than expected, here's what's actually happening and what a normal recovery looks like.

Why LASIK Causes Dry Eye

During LASIK, a thin flap is created in the cornea to reshape the tissue underneath. That flap-creation step temporarily cuts through some of the corneal nerves responsible for two things: sensation, and triggering your normal blink and tear-production reflex. With those nerves temporarily disrupted, your eyes may produce fewer tears and blink less often than they did before, even though nothing is "wrong" in the way it might feel.

How Common Is It, and How Long Does It Last?

Post-LASIK dry eye is extremely common in the first few weeks, and many patients notice some degree of it. For most people, symptoms peak in the first month and gradually improve as the corneal nerves heal, typically resolving within 3 to 6 months. A smaller number of patients experience symptoms that persist longer, especially those who had dry eye tendencies before surgery that went unnoticed.

What It Feels Like

Gritty, sandy, or scratchy sensation

Eyes that feel tired or strained

Fluctuating or blurry vision

Light sensitivity

Occasional excessive watering

What Helps During Recovery

01
Preservative-free artificial tears

Used consistently rather than only when symptoms flare.

02
Punctal plugs

Which help retain your eye's own natural tears for a period of time.

03
Prescription anti-inflammatory drops

For cases where inflammation is prolonging the dryness.

04
Addressing meibomian gland function

Healthy oil glands in the eyelids are a major part of a stable tear film.

Not everyone needs every option above. What actually helps depends on what's driving the dryness in your specific case, which is exactly why a proper evaluation matters more than guessing with over-the-counter drops alone.

When to Ask for a Closer Look

If dryness is still significantly affecting your daily comfort or vision beyond the 3 to 6 month mark, or if it feels like it's getting worse rather than better, that's a reasonable point to ask for a more structured evaluation rather than continuing to manage it on your own. A framework-based assessment, the kind aligned with the TFOS DEWS III diagnostic approach, can help identify exactly which factors are contributing, rather than treating "dry eye" as one single thing.
